Step 1:

Eligibility | File the Petition

 

You must be a resident of Miami-Dade County to file for a name change.

Filing the Petition:

Complete the necessary forms, including the Petition for Change of Name (Adult or Minor) and the Civil Cover Sheet.

Make an appointment with the Family Court Self-Help Program to review and notarize your documents or hire an attorney to file for you.

 

Step 2:

Get Live Scan Fingerprint Service:

Step 3:
Go to Court , File Notarized Petition and Pay Fees

File the original documents with the Clerk at 175 N.W. 1st Avenue, 24th Floor, and pay the filing fee of $401.001 

You also have to pay $60.00 fee to the courts.

If you chosen the self-help option, please go to the 24th floor and file your petition after your fingerprints.

Register Online Miami-Dade Courts E-Portal

After you submit your documents, pay the filing fee, and file with the courts, you must register through the Miami Dade Courts E-portal to receive correspondence and emails from the courts.

Name Change Request Reviewed By County Judge

Adult Name Change Final Hearing

If you used the self-help program, you will not have to appear in court. The courts will correspond with you via email. If your name change is granted by the judge, you will be notified by email through the court system.

You will receive a date for your final hearing by mail, typically within 4 to 8 weeks. 

On the day of the hearing, bring your Florida Driver’s License, Florida ID, or Florida Voter’s Registration Card1.

After the hearing, obtain certified copies of your Final Judgment from the Clerk’s Office1.

If your fingerprints are rejected, you will be notified by the Licensing Agency, Employer or Courts. You may need to schedule another appointment and pay the associated fees again.

The attorney must file with the courts on your behalf on the 12th Floor (Family Court Division), they will have to your information via the Attorneys eportal from Miami Dade Courts.

No, the petition must be started through the miami court system, you will receive a case number that will be used with your fingerprint file.
