Adult Name Change process For Miami-Dade County
Step 1:

You must be a resident of Miami-Dade County to file for a name change.
Filing the Petition:
Complete the necessary forms, including the Petition for Change of Name (Adult or Minor) and the Civil Cover Sheet.
Make an appointment with the Family Court Self-Help Program to review and notarize your documents or hire an attorney to file for you.
Step 2:
Get Live Scan Fingerprint Service:
A background check must be performed. You need to get Live Scan fingerprinting services. Florida ORI Number FL2394OZ
Step 3:
Go to Court , File Notarized Petition and Pay Fees
File the original documents with the Clerk at 175 N.W. 1st Avenue, 24th Floor, and pay the filing fee of $401.001
You also have to pay $60.00 fee to the courts.
If you chosen the self-help option, please go to the 24th floor and file your petition after your fingerprints.
If you have selected an attorney to file on your behalf, the attorney must go to the 12th Floor Family Court Department to file.
Step 4:
Register Online Miami-Dade Courts E-Portal

After you submit your documents, pay the filing fee, and file with the courts, you must register through the Miami Dade Courts E-portal to receive correspondence and emails from the courts.
